I meet with a therapist about once a week, mostly to help me handle problems at my job better. Recently, my boss told me that some coworkers think I don't care about my job. That's not true at all, although my anxiety over things that are going on might make it appear that way; idk. I'm thinking about asking my therapist if she would write my boss a letter just to attest that we've been trying to work on work issues in therapy for several months now. I realize that's a breach of confidentiality, but I thought that might be ok if I consented to it. Do you think this would help my supervisor to see that I am invested in the job, or would it just make me look crazy?

With your permission - absolutely. Your boss would have a better understanding what's going on, and that you try to get better. It also gives him the opportunity to be supportive, he might even offer you flexibility and adjustments that allow you to thrive for years to come.

I think you should go in and TALK to your supervisor yourself. If you are old enough to have a job, you are old enough not have someone write you a note. As a manager I would not put much weight on a note from you doctor, I put weight on performance and attitude.

No way in hell would I tell my employer I had a mental illness and saw a therapist. If you want to tell them something, say you have a MEDICAL issue that you're working out. Nothing major, just requiring some trial and error to resolve.

Absolutely this!
What goes on between you and your therapist is your business! Yes, your therapist can write a letter on your behalf, but it is against Federal HIPAA law for your supervisor or any employer to hold your condition against you. Moreover, you DO NOT have to tell your employer your specific conditions.
Your therapist can write a letter requiring a leave of absence from your job; you show your HR people that letter and they can honor it. If they ask, just tell them "medical reasons", and that's it. Do NOT go into detail - it is none of their business. And stop giving information to your supervisor. Whatever you say can be used against you later on.

Thanks for the input, everyone. I don't think I will ask my therapist to write such a letter, now that I've read your replies and had time to reflect on it. I would if I needed a leave of absence (details excluded) but with any luck, I don't think it'll come to that. Thanks again.
